Job Title : HCA Lead Aide
FLSA Status : Non-Exempt

Job Summary: Performs patient care activities in the community and at the Adult Day Health Center. Will assist with scheduling and assignments. Will assist with training and mentoring of CNA/PCA staff. Covers absences and Start of Care Services. Will update team members. Reports directly to the Home Care Agency Supervisor.
SpecificationsEducation : High school diploma or equivalent. Currently listed as NA1 or NA2 with NC Division of Facility Service with no substantiated findings of neglect, abuse, or misappropriation of resident’s property in a nursing home.
Experience : 1-2 years experience as a nursing tech/nursing aide, or EMT or corpsman. At least 1 years experience working with the frail elderly population.
Number and Type of Employees Supervised (optional) : None
Licensure, Registry or Certification Required : Valid NC driver's license and vehicle. Certificate of completion from a nursing tech/aide program.
Special Training : Good driving record; able to communicate effectively both verbally and in written form; able to position and turn participants; knowledge of and/or care of adults with chronic health care conditions. Only act within the scope of his or her authority to practice. Meet a standardized set of competencies for the specific position description established by PACE@Home and approved by CMS before working independently.
Immunizations: Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact

Key Responsibilities : (*denotes an age-related skill or task)
· Provides personal care (bathing, dressing, feeding, toileting, transfers, and incontinent care) to participants of PACE@Home in the Home and in the Adult Day Health Center.
· Responsible to report changes to Home Care Agency Supervisor.
· Supports PACE@Home’s mission to be the champion for seniors wishing to remain in their community; its vision to be the preferred provider of individualized care for seniors in the community; and its values of respect, integrity, accountability, compatible goals, and compassionate care.
· Documents services provided and observations on appropriate forms.
· Participates in the on-call process.
· Provides routine rounding times for CNA members on the team.
· Will be flexible in hours worked to accommodate the needs of the participants in the homes. Ex: Cover for Vacations or callouts etc.
· Other duties as assigned.
