Pride Health is seeking a LPN Home Health for a 13-week contract (with high possibility of extension) at a healthcare facility in Palm Bay, FL . This is a great opportunity for experienced candidates to work in a fast-paced healthcare environment while gaining valuable experience.

Position Details:

• Job title: LPN Home Health
• Location: Palm Bay, FL
• Start Date: 10/13/2025
• Contract Length: 13 weeks
• Shift: Days | 5X8s
• Hours per Week: 40 hours
• Travel Pay: $1,660/week

Key Responsibilities:

• Provide skilled nursing care to patients in their homes under the supervision of an RN and in accordance with the care plan.
• Administer medications, treatments, and wound care while monitoring patient responses and reporting changes in condition.
• Educate patients and families on disease management, medication administration, and self-care techniques.
• Document all care, observations, and interventions accurately and timely per agency and regulatory requirements.
• Collaborate with physicians, RNs, and other healthcare professionals to ensure safe, effective, and coordinated patient care.

Experience:

• 2 year's experience as an LPN in a clinical setting, preferably in a home health or hospice setting
• Homecare Homebase Software Experience
• OASIS Experience

Certifications & Skills:

• Active LPN License
• Valid Driver's License
• BLS (AHA)

*Offered pay rate will be based on education, experience, and healthcare credentials.
