Join a growing outpatient Community Clinic, focused on improving the lives of underserved communities.

• Team-based collaborative community outpatient practice with a team of Family physicians, NPs, RNs, Pediatricians, OB-GYNs, Dentists, specialists (optometrist, podiatrist, dermatologist), LCSWs, mental health professionals, case managers, health educators, registered dietitians, LVNs. 
• Strongly prefer bilingual in  English/Spanish to deliver care in Spanish and English.
• Committed to providing high-quality care to underserved patients
• Primary care and chronic disease management comprises the bulk of care.
• Responsible for assessment, diagnosis, and management of healthcare needs, utilizing standardized and evidenced-based procedures.
• EPIC EMR

Details:

• Full-time: 4 or 5 days/week
• Monday - Friday, 8AM – 5PM
  • It does require working one Saturday / month for which a weekday is flexed.
• Outpatient
